1. The applicant and agent

Please provide your contact details. You will note that your personal information covering telephone numbers and your email address is in a separate section at the end of the form. This will enable us to keep it separate because we aim to remove this before we publish details of the application on the website.

The agent is any person acting for you. If you provide details of an agent we will contact that person if we need any more information and will send all our letters, including our decision, to them unless you give us other written instructions.

2. Address of proposed development

Please tell us clearly the location of the site, giving a full address and postcode. If the land has no address, please describe the site as clearly as possible in relation to any known addresses or landmarks nearby.

The site area should be given, in metric units (metres or hectares)

3. Your interest in the land

If you are not the owner of the land, or a leaseholder with at least seven years remaining on your lease, you will need to tell us who is the owner of the land and notify them of the works that you are proposing.

4. Tree Preservation Order number (if applicable)

A Tree Preservation Order (TPO) can cover an individual tree or group of trees. If the tree is covered individually please give the reference number of the TPO; if the tree is covered in a group, area or woodland please provide the TPO numbers and a sketch map showing the location of the trees and indicate the tree(s) to which works are proposed.

5. Details of  the proposal

You must have consent for any works to a protected tree – this will include cutting it down, digging up roots, trimming or cutting branches, cutting the trunk and reducing the height of the tree(s).

Please give details of the work that you want to carry out to the tree(s) and your reasons for wanting to do this. If you want to cut off branches, please tell us which branches are involved; if you are proposing to cut off many branches please indicate the size of the crown.

6. Replacement trees 1

If you want to cut down a tree we will normally ask you to replace it with another one or more of a suitable species. Please give details of the species, size and location of the replacement tree(s) that you will plant.

7. Replacement trees 2

If you do not wish to plant a replacement tree please explain the reasons why.
